SCHOOL ID IS NOT AN ACCEPTABLE FORM OF ID

14+ services available to book:

Consults, jewelry inserts, jewelry removals, nostril(s), lobe(s), helix, conch & septum.

Multiple piercing appointments can be booked but there will be restrictions based on age requirements of individual services.

16+ services available to book:

Consults, jewelry inserts, jewelry removals, nostril(s), lobe(s), helix/flat, septum, forward helix, tragus, conch, labret, daith, industrial, faux rook, rook, navel, eyebrow, side lip, vertical philtrum, vertical labret, philtrum, and monroe.

ID Required:

• The minor getting pierced is required to have a valid state issued photo ID for themselves.

• The parent/guardian is required to have a valid state issued photo ID for themselves.

• Last names must match on photo ID’s.

• If last names do not match a birth certificate and/or name change paperwork is also required. All paperwork must be state issued.

• If a legal guardian is accompanying the minor, guardianship papers issued by the state must be present.
  Power of attorney is not sufficient unless “without limitations” or “elective cosmetic procedures” are specified on the document. Medical power of attorney does not include consent for elective cosmetic procedures.

ID Required:

• The parent/guardian is required to have a valid state issued photo ID for themselves.

• The minor getting pierced is required to have a valid state issued photo ID or their original birth certificate.

• Parent/guardian last name must be the same on Photo ID and/or birth certificate. If last names do not match name change paperwork is also required.

• If a legal guardian is present, guardianship paperwork issued by the state is required.
